To apply, visit our application page and click the pink button on the first screen. You’ll need to download, print, and mail the application to the address provided in the packet. Be sure to include a signed doctor’s release, which can come from either your primary doctor or your treating oncologist. You’re also welcome to share your story and photo by mailing them with the packet or emailing them to communications@sendmeonvacation.org.

What Happens After Applying:
Once we process your application, you’ll be eligible to request a freestyle vacation certificate or join a group vacation. We’ll send travel invites to all our group vacations throughout the year from communications@sendmeonvacation.org, or you can check our website for available trips. Vacations are subject to resort availability and resort sponsorship.

Application Status:
You only need to apply once. Your application will remain on file unless you request to have it removed.

All Applicants:
All applicants are responsible for covering their airfare, transfers, and any other specified charges on the invitation.

Payment Details:
A deposit date will be provided, and full payment is due 60 days before your departure. Travel insurance is mandatory for all participants.

Ongoing Invitations:
Once your application is complete and processed, you will be eligible to attend any upcoming vacations and will receive invitations by email. Your name will remain on our list indefinitely, and each year you’ll receive emails about new trips available for application.

Once you have traveled with Send Me On Vacation, you will receive an email invitation from communications@sendmeonvacation.org  to all our upcoming vacations.

1. Breast Cancer Diagnosis:
   The applicant must be diagnosed with breast cancer and have completed treatment.
2. Self-Application:
   The application must be submitted by the applicant themselves, not a friend or family member.
3. Medical Approval:
   The applicant must confirm that the vacation is safe and approved by their healthcare provider.
4. Personal Story:
   The applicant must provide a personal story explaining why they believe their circumstances qualify them for a vacation through Send Me On Vacation.
5. Required Documents:
   The applicant must complete, sign, and send the following documents:
   • SMOV Application Form
   • Contact Information
   • SMOV Vacation Preferences
   • SMOV Agreement
6. HIPAA Form:
   The applicant must complete, sign, and send the HIPAA (Health Insurance Portability and Accountability Act) form.
7. Physician’s Consent:
   The applicant’s physician must complete and return the Medical Statement and Consent to Participate form to Send Me On Vacation.
8. Photographs:
   Please send three current, high-resolution photographs of the applicant by email (in .jpeg or another high-quality format). Do not fax or mail the photos. Email them to communications@sendmeonvacation.org.

Unlike any other disease, fighting breast cancer is not a one-in-done process. Most of our recipients are involved in years of treatment with continuous progression. Our Vacation Ambassador Program encourages past recipients to build upon their path of emotional healing by creating a sisterhood of love and support during the fight. Living beyond breast cancer often involves numerous physical surgeries, chemotherapy, radiation and drug therapy that seems to never end. When survivors can unite and can share their current challenges, they can support and heal together at a faster and stronger rate. The combination of infusing joy, adventure and sisterhood is supported by our powerful Vacation Ambassador Program.

Once you complete your first vacation (group or freestyle), you are eligible to become a VACATION ANGEL and attend unlimited vacations at a subsidized VACATION ANGEL PROGRAM RATE.
The VACATION ANGEL PROGRAM is based on survivors uniting and sharing their current challenges with other survivors at subsequent vacations at a reduced rate. These vacations are being offered at various times and locations throughout the year based on availability, vacation donations, and interest.  Please be sure to follow all social media on Facebook and Instagram as well as save communications@sendmeonvacation.org in your email contacts.  You will not be removed from our list unless you request to be removed.
 
Travel insurance is mandatory for all participants traveling outside the USA.
 
Ongoing Invitations with The Angel Vacation Program:

Once you have completed your first vacation (group vacation or freestyle certificate), you will automatically be considered a VACATION ANGEL and eligible to attend any upcoming vacations at the VACATION ANGEL PROGAM RATE and will receive ongoing invitations throughout the year.
