How to Apply
A signed doctor’s release from either your primary care physician or your treating oncologist is required. A short (200-300 word) personal story describing your cancer journey along with a photo of yourself is also part of the application.

After You Apply
Once your application has been processed, you will be added to our database. From there, you’ll begin receiving invitations to attend our group vacations and will be eligible to request a Freestyle 7 night/8 day Vacation Certificate as your first trip at no cost to you.
You will not receive a confirmation when your application is received, but you are welcome to email us at any time to verify your status. You only need to apply once.
